JOB QUALIFICATIONS/REQUIREMENTS
Must provide evidence of a Personal Support Worker (PSW) certificate or meet the qualification outlined in Section 47 of the Long-Term Care Homes Act, 2007. An interest in working with the elderly and a general understanding of their needs is essential. Must be able to perform essential duties of the job in accordance with physical demands for the position.
JOB SUMMARY
Under the direct supervision of the unit Registered Nurse, the PSW provides personal nursing care for Residents within the Home consistent with the Home’s standards. As a member of the multidisciplinary team staff collaborate to ensure holistic individualized care is provided in accordance with the Resident’s plan of care. PSWs participate in the nursing process by providing assistance with daily living, reports all changes and concerns to the RN and records care given according to documentation procedures of the Home. The PSW ensures safe and ethical care by adhering to all Home’s policies and Ministry standards.
PRINCIPAL RESPONSIBILITIES
A. NURSING CARE
1. Participates in nursing team report at the beginning of each shift.
2. Responds to the Resident care needs as instructed by the RN which may require flexibility to meet sudden changes of assignment.
3. Organizes daily work assignment to make best use of time and carry out Resident’s care plan for assigned Residents with respect for Resident’s rights.
4. Provides direct personal care to all Residents as outlined in the individual’s plan of care, performing and/or assisting the Resident to complete the activities of daily living functions, such as; bathing, oral hygiene, grooming, toileting, ambulation, positioning, transferring, etc.
5. Documents all care provide in accordance with the Home’s policies and Ministry Standards.
6. Provides input into the evaluation of Resident care to assist in maintaining a current plan of care which meets the Resident’s needs. Provides information at the request of the RN for monthly nursing reviews of the Resident’s care.
7. Assists with the orientation of new Residents to the Home.
8. Participates in Resident/nursing care conference when requested.
9. Participates as a nursing team member by supporting, assisting, relieving and covering for other staff promptly and when required.
10. Accompanies Residents to community appointments as necessary.
11. Maintains a clean work area and a unit environment conducive to providing quality care and effective communication.
12. Encourages Resident and family to participate in the Resident’s activities of daily living and to participate in providing support which is outlined in the plan of care.
13. Participates in the serving and collection of food, assisting Residents as required to open cartons and get set up, and to feed Residents who require assistance.
14. Participates in the care of Residents after death.
15. Assists with Residents during palliative care.
16. Assists with admission, transfer and discharge of a Resident.
17. Ensures meeting all standards of care as outlined in the Ministry standards for Resident care.
B. LEADERSHIP ACTIVITIES
1. Supports and maintains an environment accepting of change.
2. Meets the ethical and legal requirements of the Ministry standards for Resident care.
3. Participates in the orientation of new nursing employees.
4. Acts as a Resident advocate by protecting the Resident’s right to:
a) access to valid information
b) informed and voluntary consent to care, treatment and participation in nursing education
c) privacy and confidentiality
d) treatment with dignity
e) participation in decisions affecting his or her care
5. Participates in unit team meetings to examine nursing programs and delivery structures for the purposes of maintaining cost effectiveness and nursing efficiency.
C. EDUCATION, HEALTH AND SAFETY
1. Is responsible for maintaining knowledge of policies and procedures current for one’s nursing responsibilities.
2. Attends meetings, educational programs, and/or inservice programs offered by the employer to enhance ability to deliver quality care.
3. Complies with infection control policies and procedures.
4. Complies with Fire and Safety Regulations and attains proficiency in the method of sounding fire alarms, fire evacuation procedures, proper use of fire extinguishing equipment and other procedures designed to ensure the security and protection of the Residents of the Home.
5. Works in compliance with the provisions of the Occupational Health and Safety Act to maintain a safe working environment.
6. Reports all accidents and/or safety concerns to the supervisor immediately.
D. OTHER
1. Co-operates with other departments while performing daily duties. Nursing is expected to:
. assist activation in portering to and from activities and in Resident exercise/therapy routines
. assist actively at meal times and on the unit to ensure Residents receive all meals and nourishments
. put away Resident clothing after it is delivered to the floor
2. Complies with government regulations which apply to the Home.
3. Performs other job related duties as assigned.
