Registered Practical Nurse – Princess Court – Dryden, ON

JOB QUALIFICATIONS/REQUIREMENTS

Provides evidence of registration with the College of Nurses of Ontario as a Registered Practical Nurse or Registered Nurse by providing a current certificate of registration on hiring and annually thereafter by February 15 of each year.  Provides evidence of pharmacology preparation as set out in Ministry Standards for Long Term Care.  Provides evidence of Aseptic technique preparation.  Foot care certification preferred.  Provides evidence of current Heart Saver C.P.R. certification and every three years, thereafter.  Demonstrates excellent interpersonal skills and effective communication skills in both written and verbal form.  Computer knowledge would be an asset.  Experience in gerontological nursing would be an asset.  A genuine interest in the elderly with an understanding of their needs is essential.  Must be able to perform essential duties of the job in accordance with physical demands for the position.  Must pass a criminal reference check.  Must provide evidence of tuberculin testing as per Ministry Standards.

JOB SUMMARY

Under the direct supervision of the unit Registered Nurse provides nursing care for Residents within the Home consistent with the Home’s standards.  As a member of the nursing and multidisciplinary teams, collaborates to ensure holistic individualized Resident care.  In consultation with the Registered Nurse (RN), the Registered Practical Nurse (RPN) uses the nursing process as established by the College of Nurses of Ontario in the current Standards of Nursing Practice for Registered Nurses and Registered Practical Nurses and the policies and procedures set out by the Home.  The RPN participates in the assessment, planning, implementation and evaluation of Resident care within one’s scope of practise to ensure delivery of safe, effective and ethical nursing care.  The scope of practise for the RPN as set out by the College of Nurses may vary from the expectations for practise as set out in the policies and procedures of the Home.  In all such cases, the Home’s policies must be followed.

PRINCIPAL RESPONSIBILITIES

A.        NURSING CARE

 1.        Participates in nursing team report at the beginning of each shift.

 2.        Demonstrates professional knowledge and skill in daily practise by responding quickly and efficiently to unexpected changes in work volume, emergencies, staffing and scheduling changes.

 3.        Organizes and prioritizes daily work assignment at outset of each shift.

 4.        Carries out the individual Resident’s plan of care (master care plan) by competently performing basic nursing skills in compliance with current standards of practice for RPN’s as specified by the College of Nurses of Ontario.

 5.        Evaluates and documents the Resident’s condition and response to nursing care.  In consultation with the Registered Nurse, adjusts Resident care as necessary to accommodate the Resident’s changing status.

 6.        Contributes data for evaluation and monthly reviews of Resident care plans and documents according to Home’s standards.

 7.        Performs assigned treatments and documents nursing actions and observations.  Recognizes and reports symptoms of adverse reaction to treatment, reports declining conditions to the RN and documents same on the Resident record.

 8.        Provides health teaching to Residents and families/significant others as outlined in Resident’s plan of care.

 9.        Administers the medications program as assigned and documents medications given on the Resident records.

10.       Consults with the RN for all PRN medications.

11.       Contributes assessment data to the RN regarding the effectiveness of the Resident’s medication.

12.       Assists the RN as required with acutely and/or terminally ill Residents.

13.       Assists with the orientation and direct care to new Residents.

14.       Contributes to the multidisciplinary assessment process and provides assessment data to the RN during the first 24 hours after admission.  This includes communicating concise and appropriate information concerning a Resident’s needs and overall condition to the RN and other members of the multidisciplinary team.

15.       Participates in Resident/nursing care conferences when requested.

16.       Participates as a nursing team member by supporting, assisting, relieving and covering for other staff members promptly and when required.

17        Accompanies Residents to community appointments as necessary.                                     

18.       Provides direct care to all Residents as outlined in the individual’s care plan, performing and/or assisting the Resident to complete the activities of daily living functions, such as, bathing, oral hygiene, grooming, toileting, ambulation, positioning, transferring, etc.

19.       Maintains a clean work area and an environment conducive to providing quality care and effective communication.

20.       Encourages Resident and family/significant others to participate in the Resident’s activities of daily living and to provide support which is outlined in the plan of care.

B.        LEADERSHIP ACTIVITIES

1.         Supports and maintains an environment accepting of change.

2.         Meets the ethical and legal requirements of professional practice in all independent and dependent functions.

3.         Documents data in the progress notes, when necessary, for Residents receiving care from others who do not have documentation authority in the nursing progress notes.

4.         Participates in the orientation of new nursing employees.

5.         Acts as a Resident advocate by protecting the Resident’s right to:

a)         access to valid information

b)         informed and voluntary consent to care, treatment and participation in nursing education

c)         privacy and confidentiality

d)         treatment with dignity

e)         participation in decisions affecting his or her care

6.         Participates in unit team meetings to examine nursing programs and delivery structures for the purpose of maintaining cost effectiveness and nursing efficiency.

C.        EDUCATION, HEALTH AND SAFETY

1.         Is responsible for maintaining a current certificate of registration and demonstrates and maintains that competence at a current level of knowledge for one’s practise including added skills required.

2.         Attends meetings, educational programs, and/or inservice programs offered by the employer to enhance ability to deliver quality care.

3.         Complies with infection control policies and procedures.

4.         Complies with Fire and Safety Regulations and attains proficiency in the method of sounding fire alarms, fire evacuation procedures, proper use of fire extinguishing equipment and other procedures designed to ensure the security and protection of the Residents of the Home.

5.         Works in compliance with the provisions of the Occupational Health and Safety Act to maintain a safe working environment.

6.         Reports all accidents and/or safety concerns to the supervisor immediately.

D.        OTHER

1.         Co-operates with other departments while performing daily duties.  Nursing is expected to:

  .         assist activation in portering to and from activities and in Resident exercise/therapy routines

  .         assist actively at meal times and on the unit to ensure Residents receive all meals and nourishments

  .         put away Resident clothing after it is delivered to the floor

2.         Complies with Government regulations which apply to the Home. 3.         Performs other job related duties as assigned
