This position also plays a key role in coordinating resident admissions and supporting families through the transition into long-term care.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Key Responsibilities<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Provide counselling, coaching, care planning, and therapeutic support for residents and families<\/li>\n\n\n\n<li>Support staff in the use of responsive behaviour approaches including PIECES and U-First<\/li>\n\n\n\n<li>Collaborate with the interdisciplinary team to develop and evaluate resident-centred interventions<\/li>\n\n\n\n<li>Complete psychosocial and behavioural assessments for residents<\/li>\n\n\n\n<li>Participate in resident care conferences and BSO meetings<\/li>\n\n\n\n<li>Coordinate admissions, resident tours, and orientation to the home<\/li>\n\n\n\n<li>Liaise with Home and Community Care Support Services regarding admissions<\/li>\n\n\n\n<li>Support quality improvement initiatives and maintain required reporting\/statistics<\/li>\n\n\n\n<li>Promote a safe and supportive environment for residents, families, and staff<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Qualifications<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Current registration with the Ontario College of Social Workers and Social Service Workers<\/li>\n\n\n\n<li>Experience working within a health care setting<\/li>\n\n\n\n<li>Demonstrated leadership, communication, and interpersonal skills<\/li>\n\n\n\n<li>Knowledge of dementia care, responsive behaviours, and long-term care practices considered an asset<\/li>\n\n\n\n<li>Experience with Behavioural Supports Ontario programming is preferred<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Why Join Us?<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Opportunity to make a meaningful impact in the lives of seniors and families<\/li>\n\n\n\n<li>Collaborative interdisciplinary work environment<\/li>\n\n\n\n<li>Supportive leadership team<\/li>\n\n\n\n<li>OMERS pension plan<\/li>\n\n\n\n<li>Competitive compensation and benefits package<\/li>\n<\/ul>\n\n\n\n<p>Interested applicants are encouraged to submit a resume and cover letter outlining their qualifications and experience.<\/p>\n\n\n\n<p>The District of Kenora Homes for the Aged is committed to inclusive and accessible employment practices.
