Admission Process
Admission inquiries for all long-term care homes in the province of Ontario are initially directed to Ontario Health atHome.
An Ontario Health atHome Care Coordinator will conduct an assessment of the prospective applicant(s) and work closely with the applicant(s) or substitute decision maker(s) throughout the application process. The local Ontario Health atHomr office manages the waiting lists for the homes.
Over our many years of kind and caring service, we have found our new residents like to know what to expect on the day of admission and what to expect in the days following. For that reason, here is a list to guide you through the process to ensure a smooth and easy transition.
- You will be notified by your local Ontario Health atHome Care Coordinator that your application to the Home has been approved, and given either a date of admission or spot on a waiting list.
- Upon arrival to the Home, the receptionist will greet you. Family and friends are encouraged to join in with you for a part or all of the day.
- You will be introduced to the Admissions Lead, who will be your personal guide through the admission process.
- You will be given help to bring in your belongings, shown your new room and introduced to your roommate if applicable.
- You will meet with your Nurse. This is a time for them to get to know you, and for you to ask any questions of them.
- Your clothing will be sent down to laundry for labeling and laundering.
- A tour of the Home and your Resident Area will be arranged with the staff to familiarize yourself with your new home.
- At a time that is convenient for you and the staff, you will be invited to complete and sign the Admission Agreement, and arrange the payment method.
- Over the course of the next few days, you will meet the Nurse Practitioner, Program staff, Dietitian and Recreation / Therapy Lead, along with many other new staff, residents and families.
- Within six weeks of admission, you and your loved ones will be invited to attend an Interdisciplinary Care Conference. At this meeting you will meet with the Home’s Health Care Team to discuss your care up to that point.
