Position Overview
The Behavioural Support Ontario (BSO) Social Worker / Admissions Lead plays a key role in supporting residents with complex and responsive behaviours, as well as guiding new residents and families through the long-term care admissions and transition process. Working collaboratively with the interdisciplinary team, this role ensures high-quality, resident-centered behavioural support and a smooth, compassionate admissions experience.
Qualifications
- Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Experience in a health care setting
- Demonstrated leadership skills
Key Responsibilities
Behavioural Support Ontario (BSO) Social Work Services
- Provide coaching, care planning, monitoring, and evaluation of therapeutic interventions related to responsive behaviours.
- Collaborate with the Management Team to implement best practices and therapeutic approaches.
- Identify resident-centered strategies and liaise with BSO community resources.
- Support staff in using frameworks such as P.I.E.C.E.S. and U-First! for assessing and managing responsive behaviours.
- Provide counselling, community linkage, and social support for residents.
- Complete assessments and develop behavioural strategies for pre-admission, admission, transitional, and episodic cases.
- Offer psychosocial counselling to residents and caregivers within the scope of the College.
- Recommend bio-psychosocial and non-pharmacological interventions.
- Complete initial PIECES assessments and other tools as required.
- Monitor and adapt care strategies in collaboration with the LTCH care team.
- Maintain therapeutic communication and establish supportive relationships with residents, families, and staff.
- Model and coach behavioural strategies for staff during escalating situations.
- Participate in care conferences, responsive behaviour meetings, and required regional BSO meetings.
- Collect and maintain service data, statistics, and reports for Quality Improvement and LHIN reporting requirements.
Resident Admissions
- Act as the primary liaison with the Home and Community Care Branch of the NW LHIN for admissions coordination.
- Review applications and make admission decisions collaboratively with the Director of Care/designate.
- Coordinate tours for prospective residents and families.
- Complete psychosocial assessments for new residents.
- Welcome and support residents and families on admission day.
- Ensure completion and distribution of all required admission documentation.
- Coordinate and assist with internal room changes as needed.
Education, Health & Safety Responsibilities
- Participate in ongoing training related to best practices and responsive behaviours.
- Adhere to Infection Prevention and Control protocols.
- Comply with all Fire and Safety regulations and emergency procedures.
- Follow the Occupational Health & Safety Act to maintain a safe working environment.
- Immediately report accidents or safety concerns to the Adminsitrator.
- Conduct monthly workplace inspections.
- Promote safety standards and ensure staff have required training and equipment.
Other Duties
- Perform additional responsibilities as assigned by the Administrator.
