JOB QUALIFICATIONS/REQUIREMENTS
Provides evidence of registration with the College of Nurses of Ontario as a Registered Nurse (RN) by providing a current certificate of registration on hiring and annually thereafter by February 15 of each year. Provides evidence of current Heart Saver C.P.R. certification on hiring and every three years, thereafter. Provides evidence of current First Aid certification and maintains certification where work environment has a First Aid Station. Demonstrates excellent interpersonal and leadership skills. Communicates effectively both in written and verbal form. Computer knowledge would be an asset. Post RN education in gerontology is preferred such as is provided in diploma or certificate courses offered through community colleges. Experience in gerontological nursing and organizational management would be an asset. A genuine interest in the elderly with an understanding of their needs is essential. Must be able to perform essential duties of the job in accordance with physical demands for the position. Must pass a criminal reference check. Must provide evidence of tuberculin testing as per Ministry Standards.
JOB SUMMARY
Under the direct supervision of the Director of Nursing is responsible for the day to day coordination, implementation, and supervision of the nursing program. Assists in the planning, organizing, evaluating and directing of nursing care programs for Residents according to established Homes and Nursing polices and procedures and Ministry standards and guidelines. Functions as a member of the nursing and multidisciplinary teams, collaborating effectively to ensure holistic individualized Resident care. Advocates for the Resident to ensure the Resident’s rights are incorporated into the Resident’s plan of care. Uses the nursing process as established by the College of Nurses of Ontario in the current Standards of Practice for Registered Nurses and Registered Practical Nurses and the policies and procedures set out by the Home in giving basic nursing care.
PRINCIPAL RESPONSIBILITIES
A. ACCOUNTABILITY
1. Using the nursing process is responsible for practising within the current standards for the Registered Nurse established by the Ontario College of Nurses and within Ministry guidelines and Home’s policies and procedures.
2. Must be knowledgeable of the Home’s administrative and nursing policies and procedures and ensures appropriate application by nursing staff.
3. Responsible for ensuring that each Resident receives the required nursing care which incorporates a multidisciplinary approach and that this care is outlined in the Resident’s master profile care plan and is appropriately documented.
4. Accountable for ensuring appropriate staffing levels and patterns with deployment and delegation of staff to meet Resident care needs.
5. Responsible for leadership, teaching and effective supervision of nursing staff in carrying out all nursing programs.
6. Ensures a safe, clean and comfortable environment for the Residents, visitors and employees of the Home by working in compliance with all legislation, policies of the Home (e.g. Infection Control practices, Safety policies, WHMIS, Fire Plan, Disaster Plan, Occupational Health and Safety Act).
7. Completes necessary documentation and reports concerns promptly to supervisor.
8. Ensures appropriate application of Resident rights and all applicable legislation (Advocacy Act, Substitute Decisions Act and Consent to Treatment Act).
B. NURSING CARE
- Coordinates the nursing team to receive and give reports at the beginning and end of each shift. Communicates clearly, concisely and effectively to ensure appropriate delivery of quality care based on Resident individual needs.
- Demonstrates professional knowledge and skill in daily practise by responding quickly and efficiently to unexpected changes in Resident care requirements, work load volume, emergencies, staffing and scheduling changes by appropriate deployment of staff and cost effective use of resources within the nursing budget to meet all identified care needs.
- Ensures that each Resident plan of care is implemented by competently performing basic nursing skills in compliance with current standards of nursing practice for RN’s and RPN’s as specified by the College of Nurses of Ontario and/or delegating nursing actions to appropriately prepared nursing staff.
- Evaluates and documents the Resident’s condition and response to nursing care. In consultation with the Registered Nurse, adjusts Resident care as necessary to accommodate the Resident’s changing status.
- Performs assigned treatments and documents nursing actions and observations. Recognizes and reports symptoms of adverse reaction to treatment, reports declining conditions to the RN and documents same on the Resident record.
- Administers the medications program as assigned and documents medications given on the Resident records.
- Evaluates the performance of supervised staff within the individual’s scope of practice in accordance with all Home’s policies and procedures; keeps supervisor informed of work progress.
- Liaises with the Resident’s physician promptly in all emergent situations and as necessary for follow-up.
- Provides health teaching to Residents and families/significant others as outlined in Resident’s plan of care.
- Assists palliative and/or end of life residents.
- Assists with the orientation and direct care to new Residents.
- At the outset of the shift on days and evenings and before shift end for nights, personally visits all newly admitted Residents, and assesses all Residents requiring professional nursing care whose conditions are not stable at the outset of the shift.
- Records changes in Resident condition describing current observations objectively and quantitatively. Documents nursing actions taken in the progress notes, and records nursing orders (follow-up care) to be taken in the Resident plan of care (master profile).
- Reports on Resident condition to multidisciplinary team as required to ensure holistic continuous care.
- Meets all documentation standards for the Home and maintains a Resident care assignment to ensure meeting all legislated requirements for maintaining appropriate Resident records.
- Ensures that all physicians’ orders are appropriately processed and effected in a timely manner.
- Ensures that all nursing programs are implemented according to the Home’s policies and procedures and Ministry requirements (e.g. Dental program, Foot care program, Medication program).
- In all aspects of service exercises professional judgement according to the Home’s and Ministry policies and legislation based on the Resident’s biopsychosocial and religious needs.
- Communicates effectively with the Resident and his or her family or substitute decision maker, the physician, and other Home’s and community personnel.
- Involves the Resident and/or family and/or substitute decision maker in planning care and helps them to understand the Resident’s condition and treatment, ensuring informed consent. Encourages and identifies in the Resident plan of care family participation in the Resident’s Activities of Daily Living (ADL).
- Conducts and/or participates in conferences and meetings of the Home.
- Provides direct care to all Residents as outlined in the individual’s care plan, performing and/or assisting the Resident to complete the activities of daily living functions, such as, bathing, oral hygiene, grooming, toileting, ambulation, positioning, transferring, etc.
- Maintains confidentiality of all Resident and Home’s records pertaining to both Residents and staff.
- Utilizes structured and unstructured time to the best advantage of quality care.
- Supports the goals and philosophy of the nursing department.
- Cooperates with all nursing units in the overall provision of quality nursing care.
- Represents the agency in a professional, courteous, and tactful manner demonstrating respect for the rights of others.
C. LEADERSHIP ACTIVITIES
1. Supports and maintains an environment accepting of change.
2. Meets the ethical and legal requirements of professional practice in all independent and dependent functions.
3. Documents data in the progress notes, when necessary, for Residents receiving care from others who do not have documentation authority in the nursing progress notes.
4. Ensures appropriate orientation of new nursing employees and any employee who has not worked in the area for some time.
5. Acts as a Resident advocate by protecting the Resident’s right to:
a) access to valid information
b) informed and voluntary consent to care, treatment and participation in nursing education
c) privacy and confidentiality
d) treatment with dignity
e) participation in decisions affecting his or her care
6. Coordinates unit team meetings and participates in nursing coordination meetings to examine nursing programs and delivery structures for the purposes of maintaining cost effectiveness and nursing efficiency.
D. EDUCATION, HEALTH AND SAFETY
1. Is responsible for maintaining a current certificate of registration and demonstrates and maintains that competence at a current level of knowledge for one’s practise including added skills.
2. Attends meetings, educational programs, and/or inservice programs offered by the employer to enhance ability to deliver quality care.
3. Complies with Infection control policies and procedures.
4. Complies with Fire and Safety Regulations and attains proficiency in the method of sounding fire alarms, fire evacuation procedures, proper use of fire extinguishing equipment and other procedures designed to ensure the security and protection of the Residents of the Home.
5. Works in compliance with the provisions of the Occupational Health and Safety Act to maintain a safe working environment.
6. Reports all accidents and/or safety concerns to the supervisor immediately.
E. OTHER
1. Cooperates with other departments while performing daily duties. Ensures that nursing will:
. assist activation in portering to and from activities and in Resident exercise/therapy routines
. assist actively at meal times and on the unit to ensure Residents receive all meals and nourishments
. put away Resident clothing after it is delivered to the floor
2. Complies with government regulations which apply to the Home.
3. Performs other job related duties as assigned.
